Summary

The Care Coordinator plays a crucial role in ensuring our care teams are deployed effectively, resources are optimized, and communication flows seamlessly between field teams, telehealth providers, and patients. You’ll manage day-to-day logistics, coordinate patient and clinician schedules, and be the hub that keeps operations running smoothly for acute-level care delivery.

Key Responsibilities

Coordinate deployment of field care teams across multiple markets based on patient needs and clinician availability.
Manage daily logistics: patient scheduling, resource planning, and operational troubleshooting.
Serve as the communication hub between field teams, telehealth providers, and patients.
- ️ Monitor & respond to field safety alerts, escalating issues in real time.
Leverage multiple software platforms to perform duties efficiently.
Assist with overflow inbound communications from patients or internal teams.
Support process improvements and quality initiatives for care delivery.
Communicate clearly and effectively with patients and providers.

Strong communication and interpersonal skills with patients and multidisciplinary teams.

Proficient in Google Suite and healthcare platforms.

✅ Requirements

2+ years of experience in a healthcare setting (ambulatory, home health, pre-hospital, etc.)
Strong documentation and communication skills
Remote-ready with ability to work independently and collaboratively
Proven ability to escalate care or operational issues appropriately
Calm under pressure with a strong sense of urgency
