We are searching for a Clinic Manager/Patient Navigator to join the Harmony Health Clinic team. This person will be a compassionate and caring individual who is fully committed to helping uninsured and underserved patients obtain free, quality health care and other needed services in order to serve their health and wellness needs.

About Harmony Health Clinic

Harmony Health Clinic seeks to understand and serve the health and wellness needs of the medically uninsured and underserved who live or work in Central Arkansas, by providing access to quality health care at no cost to these patients in a private, community-based clinic, staffed by volunteer professionals and marked by a unique atmosphere of caring, compassion, respect, dignity, and diversity.

Harmony holds weekly medical and dental clinics as well as regular specialty clinics in health coaching, dermatology, endocrinology and other services. All services are provided at no charge to patients. We provide referrals to our patients for free specialty care through a network of partnerships.

Harmony Health Clinic was founded in 2007 and is committed to advancing social justice through the provision of quality health care to those who are denied it by virtue of barriers such as socioeconomic status. We believe that universal access to decent health care is integral to the sanctity, development, and enjoyment of life, and vital to an individual’s ability to fully realize one’s dignity and potential. To date, we have provided well over $4,500,000 in services to patients.

Summary

The Clinic Manager/Patient Navigator is a 40 hour per week position with (flexible) evening and weekend hours, responsible for helping guide patients through HHC’s various clinical systems as well as providing needed external referrals for health care and a variety of social services. All positions must:

Maintain patient confidentiality; comply with HIPAA and compliance guidelines set by the organization
Attend personnel meetings, as requested

The Clinic Manager/Patient Navigator reports directly to the Executive Director and works with other personnel, members of the Board of Directors, and volunteers.

Interview required. Start date as soon as possible; position will remain open until filled. Salary is dependent on experience.

Job Description

Oversees HHC’s day-to-day clinical operations for medical and lab services
Interprets for Spanish speaking patients and/or recruiting interpreters for medical clinics, as needed
Ensures all clinics are in compliance and operating in accordance with all established licensure requirements, HIPAA requirements; organization policies and procedures, laws, rules, and regulations
Works to ensure proper scheduling and follow-up of medical patients
Provides overview of HHC and other services to patients upon arrival
Works with Dental Manager, resolves patient complaints and issues in accordance with organization policies and procedures, regulations, and applicable board standards
Works to coordinate specialist and other referrals, as needed
Assists patients with the application process for receiving care at HHC as well as applications for other providers and services
Assists patients and providers during appointments, as needed
Assists patients in understanding their responsibilities, diagnoses, treatment options, and other resources available to them
Facilitates communication between the patient, providers and any family members and friends desired by patient
Assists with patient referrals to HHC’s network of specialists offering free care to HHC patients
Develops and maintains meaningful relationships with partner organizations serving the same or similar populations, in order to help facilitate referrals
Maintains a comprehensive resource guide for patient needs, including other free and charitable clinic options, specialty referral options, other medical services, and a variety of social services
Assists patients in finding and enrolling in programs for health care and social services, such as Medicaid, AR Breast Care, SNAP, food pantries, job training, housing support, etc. as needed.
Assists patients in obtaining financial, legal, and social supports
Develops and implements patient education programs and tools, as needed
Works closely Executive Director to identify and help meet medical and other needs of Harmony’s patients
Oversight of lab - maintains compliance of lab requirements, train/recruit lab volunteers, lab budget, supply ordering, machine maintenance, and communication with Department of Health
Creates and disseminates regular reports to Executive Director and Board, including personnel, patient, and volunteer metrics
Achieves annual goals that are set along with Executive Director
Other duties as assigned by Executive Director

Qualifications

Bachelor’s Degree in Social Work from CSWE-accredited institution or experience in Healthcare preferred
Six months experience in patient navigation, social work, or nursing (can be met through internships or externships) preferred
Fluency in Spanish, particularly in medical terminology required
Knowledge of dental, medical, laboratory, and pharmacy terminology preferred
Compassionate, empathetic, non-judgmental
Ability to work in a team environment
Be even-tempered and approachable, and have excellent people skills
Ability to relate well and communicate with people from diverse groups
Ability to multi-task and juggle multiple priorities
Ability to react and adjust quickly to changing conditions
Excellent communication skills; ability to communicate effectively and articulately both orally and in writing
Proficiency in Microsoft Office Suite (Word, Excel, Outlook, PowerPoint) necessary; experience with Office 365 and Google Drive preferred
Experience preferred in the following programs, or similar programs: EagleSoft, Agastha, PioneerRX, Athena and/or other patient and clinic management programs, or ability to quickly learn basic aspects of these programs in order to schedule and attend to patient needs
