Summary

We are seeking a Referral Coordinator to join our team at RemHealth. This role is essential in providing exceptional patient service and administrative support, ensuring a smooth operation at our practice. Your contributions will directly impact our mission to help patients achieve restorative sleep.

Job Title: Referral Coordinator

Key Responsibilities:

1. Referral Intake & Triage

Review and sort incoming referrals via fax, email, and web-based inquiry platforms.
Accurately assess referral completeness and initiate contact with the referring provider or source for missing information as needed.

2. Data Entry & Tracking

Input and organize referral details into the Electronic Medical Record (EMR) system.
Maintain and update a centralized tracking log of all referral sources, statuses, and patient information to ensure timely follow-up.

3. Chart Preparation

Create/confirm comprehensive patient charts with referral documentation, demographics, and insurance verification.
Ensure charts are complete and ready for administrative scheduling, following clinic-specific protocols.

4. Coordination & Communication

Serve as the primary point of contact for referral-related inquiries, offering timely and professional responses to both internal and external stakeholders.
Communicate effectively with the administrative team to support seamless scheduling of new patients.

5. Process Optimization

Support process improvements to enhance referral workflow, reduce delays, and improve patient experience.
Identify trends or issues in referral intake and escalate to appropriate team members when necessary.

6. CRM System Management

Maintain the CRM system by ensuring timely and accurate entry of referral data and communications.
Regularly audit records for consistency and completeness.
Support improvements in CRM workflows and tagging for marketing and outreach purposes.

7. Reporting & Analytics

Generate and maintain referral analytics reports to track volume, conversion rates, and referral source performance.
Collaborate with management to identify trends, gaps, and opportunities for process improvements.
Ensure reports are delivered on schedule and accurately reflect operational activity.

Required Qualifications

EMR/EHR/CRM data entry experience
Strong work ethics, excellent communication skills with a patient-first mindset.
Ability to work independently, demonstrate accountability, and maintain a high level of professionalism.
Intermediate to advanced computer skills, including video conferencing platforms.
Strong organizational skills, attention to detail, and time management abilities.
