We are seeking a compassionate and experienced Behavioral Health & Wound Care Coordinator (LPN or RN) to oversee two essential clinical programs: behavior management and wound care. This dual role is ideal for a nurse who is organized, clinically skilled, and enjoys working across departments to drive outcomes.

Key Responsibilities:Behavioral Health Oversight:

Coordinate the Medicaid Behavioral Tier Program (Tiers 1–3)
Conduct behavioral assessments and develop individualized care plans
Ensure behavior interventions are implemented and documented correctly
Participate in care planning meetings, audits, and QAPI initiatives
Serve as point of contact for behavior-related concerns and IDT coordination

Wound Care Leadership:

Conduct and document weekly wound rounds
Perform skin assessments and develop wound care plans
Monitor wound progression and coordinate with providers and consultants
Educate nursing staff on prevention and treatment protocols
Maintain complete and timely wound care documentation in PCC

Qualifications:

Current Nevada LPN or RN license (required)
At least 2 years of experience in wound care (SNF or LTC preferred)
Behavioral health experience or knowledge of Medicaid Tier system a plus
Skilled in documentation and interdisciplinary coordination
Familiarity with PointClickCare (PCC) preferred
Wound certification (WCC, etc.) preferred but not required

Job Type: Full-time

Pay: $30.00 - $38.00 per hour

Expected hours: 40 per week
