Job Overview

Harm Reduction Nurse Care Manager-About the Role

We are seeking an experienced, compassionate, and highly adaptable Registered Nurse to bridge on-site clinical care with street-level outreach . This role is built for a hands-on nurse eager to practice in both our health center and on the ground: assessing and treating complex wounds, supporting and coordinating care for patients on medication assisted treatment (methadone, buprenorphine and vivitrol), and building trust with people who use drugs and individuals returning home from incarceration, You will collaborate with community partners and our multidisciplinary team of providers, psychiatrists, nurses, counselors, peers and navigators to deliver low-barrier, trauma-informed care directly to people who face barriers to traditional healthcare.

Key Responsibilities

Clinical Outreach & Care

Identify and participate in clinical outreach in non-traditional settings, including shelters, drop-in centers, probation offices, syringe service programs, community hubs, and other locations where patients can be reached.
Work closely with Peer Recovery and Reentry Specialists to engage individuals who have experienced medical trauma or disconnection from care
Facilitate field-based telehealth visits with medical providers to support same-day treatment initiation and access to medications (i.e. buprenorphine, vivitrol, PrEP, PeP)

Low Barrier Treatment

Provide hands-on assessment and treatment for complex wounds in both clinic and field settings, including xylazine-associated wounds and injection-related injuries.
Support low-barrier access to medication assisted treatment for opioid, alcohol, and tobacco use disorders, including prescription assistance, dispensing, dose verification, and stability monitoring,
Serve as a clinical bridge between substance use treatment, primary care, preventive care (HIV/STI), and specialty services
Deliver harm reduction education and the distribution of naloxone, fentanyl and xylazine test strips, sterile syringes, and other harm reduction supplies.

Care Coordination

Partner with Correctional Health Services and reentry partners to ensure immediate healthcare continuity (primary care, substance use and medication assisted treatment, psychiatry, and other essential healthcare) following release.
Provide high-touch care coordination for patients on MAT with complex medical and social needs through diagnostic testing, emergency transitions, and referrals for specialty care, residential treatment, and home health services.
Collaborate with Patient Navigators to address housing, food, transportation, legal, benefits, and other social needs.

What We Are Looking For

New York State Registered Nurse (RN) license.
Strong clinical assessment and care coordination skills.
A minimum of two years experience in at least one area: wound care, substance use treatment, harm reduction and community-based nursing.
Comfort delivering field and street-based care and working with marginalized, unhoused, or justice-involved populations
Ability to work a flexible schedule, including evenings and weekends, to support community-based and low-barrier care.
Bilingual in English/Spanish a must.

Why This Role Is Different

This is not a traditional clinic-based nursing position. The NCM will have the opportunity to take nursing care into the community, develop meaningful relationships with patients, work alongside peers and reentry specialists, provide hands-on wound care, and help people access treatment during some of the most important transitions in their lives.

The position is ideal for a nurse who believes that where a person is, when they are available, and what barriers they face should not determine whether they receive high-quality healthcare.
